To Dr. Amanda Adams’ Patients

We are writing to inform you of an important update regarding your ongoing care at Markham Medical Centre. As you may know, Dr. Amanda Adams has been on medical leave. Unfortunately, she will not be returning to the office. However, she has arranged for a replacement to take over her practice and continue your care.

We are pleased to introduce Dr. Shironisha Sritharan who will be starting in January 2026. Dr. Adams will be supporting Dr. Sritharan over a 2-month transition period and the planned date for her to take over my practice is March 1, 2026.

With the upcoming transition, it will be necessary for you to enroll as a patient with Dr. Shiro and complete a new patient enrollment form for our Family Health Organization (FHO) for yourself (and your child(ren) under 16 years if applicable). Please click here to access the enrollment form.

Please complete and sign the form but do not enter a date as it will need to be added only after we have an official start date for Dr. Shiro from the Ministry of Health.

We certainly hope you do enroll and would encourage you to return the form promptly. It is also important to be aware that if you do not complete and return the enrollment form by the end of August 2026, it will be understood that you no longer wish to remain a patient at Markham Medical Centre.
