Annual Block Fee 2026/2027

Dear Patient,

It is time to sign up for our optional Annual Block Fee Program for Uninsured Services.

While we are proud to provide publicly funded health care, there has been a significant increase in demand for services not covered by OHIP. These services require additional physician and staff time and resources, and due to the costs of operating a medical practice, a fee must be charged accordingly.

By enrolling in our Annual Block Fee Program, you may save money if you require multiple uninsured services throughout the year. A list of services covered or offered at a discounted rate under the program is included with this letter. 

Enrollment is valid from November 1, 2026, to October 31, 2027. We send out a reminder email each fall to allow you to re-enroll for the following year. If you choose not to enroll in the program, you may still access uninsured services on a pay-per-use basis at the time the service is provided.

To submit payment, you may:

Block Fee Rates:

  • Individual: $175.00
    Couple: $255.00

  • Family: $290.00

Please note: We will not be using Doctor Care for this program this year. Please direct all payments and inquiries to our office. 

Important: Coverage applies only to uninsured services provided by your family doctor. A fee may apply if the service is provided by another physician, such as a covering doctor. 

Enrollment is entirely optional. This letter has been sent to all patients to explain our policy regarding uninsured services. If you are no longer a patient or received this letter in error, we apologize. Please also notify our staff if your contact information or health card details have changed.
