Annual Block Fee 2025/2026

 Dear Patient,

It is time to sign up for our optional Annual Block Fee Program for Uninsured Services.

Please note: You can access the full version of our newsletter (by clicking here), which includes detailed information about our policies and procedures, a list of covered services, and answers to frequently asked questions. Additional information is also available on our website.

We are proud to provide publicly funded health care. However, over time, there has been a significant increase in the demand for services that are not covered by OHIP. These uninsured services require considerable time and resources from both physicians and staff. Due to the financial realities of medical practice, we are unable to offer these services free of charge and must collect compensation accordingly.

By enrolling in our Annual Block Fee Program, you may save money if you require multiple uninsured services throughout the year. A list of services that are either fully covered or offered at a discounted rate under this program is included with this letter.

Enrollment is valid from November 1, 2025, to October 31, 2026. We send out a reminder email each fall to allow you to re-enroll for the following year. If you choose not to enroll in the program, you may still access uninsured services on a pay-per-use basis at the time the service is provided.

To submit payment, you may:

Block Fee Rates:

  • Individual: $165.00
    Couple: $245.00

  • Family: $280.00

Please note that we will not be using Doctor Care for this program in the upcoming year. Kindly direct all inquiries and payments to our office directly.

Important: Coverage under this plan applies only when uninsured services are provided by your family doctor. If you receive an uninsured service from another physician- such as a covering doctor while your family physician is away- a fee may apply.

This letter has been sent to all our patients to inform them of our office policy regarding uninsured services. Enrollment in the Annual Block Fee Program is entirely optional. If you have received this letter in error, or if you are no longer a patient at our clinic, we apologize. If your contact information or health card details have changed, please notify our staff.
